HMO – Provided by Cigna & Fully Insured

• Must choose from list of CIGNA HMO dental providers
• No calendar year maximum
• No deductible Reduced, fixed charges for all covered services

 


PPO – Provided by Allegiance & Self Insured

• May go to any dentist of your choice..
• $1,500 calendar year maximum
• $50 deductible per person
• Plan pays 80% of preventative, 80% of Basic and 50% of Major for covered dental services
• No orthodontics
